Pathologic Analysis of the Resected Stomach in Patients with Early Gastric Cancer

Abstract
Total 35 cases(37 lesions) of early gatric cancer operated at Chonbuk University Hospital for two years from January 1991 to December 1992 were examined and analyzed to evaluate the relationship among age, sex, gross type, histopathological type,
lymph
node metastasis, and Kodama's growth pattern.
@ES The results were summarized as follows:
@EN 1. The proportion of patients with early gastric cancer(EGC) of all gastric cancer was 22.3%. Male to female ratio was 4:1 and the mean age of EGC patients was 53.8 years.
2. The proportion of sites involved by EGC in the stomach was as follows : the antrum and pylorus 67.7%, body 27.0%, and cardia and fundus 5.4%. And the incidence of gross types was as follows in order of frequency : excavated type(75.7%),
protruded
type(16.2%), and flat type(8.1%).
3. The size distribution of the EGC was as follows : 59.5% of the lesions was smaller than 2.0cm, 37.8% was between 2.0cm and 5.0cm, and 2.7% was larger than 5.0cm. Eight lesions out of nine protruded and flat types were smaller than 3cm in
greatest
diameter.
4. Histologically, protruded and flat types, showing well to moderate differentiation in eight out of nine cases, exhibited better differentiation than excavated type. Submucosal invasion was found in 22.2%(2/9) of the protruded and flat types
and
in
64.3%(18/28) of the excavated type.
5. On Kodama's classification, the incidence of growth pattern of EGC was as follows in order of frequency : small mucosal type(54.1%), pen type(27%), super type(16.2%), and mixed type(2.7%).
6. Protruded and flat types revealed growth pattern of small mucosal type and the excavated type revealed growth patterns of small mucosal, super, pen, and mixed type according to Kodama's classification. Histologically, lesions of pen type and
mixed
type exhibited less differentiation than those of small mucosal and super type.
7. Four out of 35 cases(11.4%0 had regional lymph node metastases. All of them showed submucosal invasion and showed growth pattern of pen type based on Kadama's classification.
